Insurance Coverage & Fees

Your questions about our fees and payment options are always welcome. Crystal Plastic Surgeons participates in most major insurance plans, managed care organizations (HMOs and PPOs), Medicare and Medicaid. Specific coverages may vary, so patients are encouraged to discuss particulars with us before treatment begins. When insurance coverage applies, we will prepare and submit your insurance claims for you. For elective and uncovered procedures, we provide other payment options, including Visa, Mastercard, Discover and CareCredit Financing.
